Coould be a hyd failure, dirty fluid in the clutch hydr will cause poor shifting, missed shifts, clutch pedal sticking to the floor. Worn clutch will usually slip/overev in higher gears, climbing hills or under accel, may smell slight burning. At your mileage,if you are going to do one,do both(clutch assy and hydr TO bearing.

change your clutch fluid and trans fluid. basically one of the two is old and your clutch isnt fully disengaging, or your trans is gummy inside as well. also the possibility of a bent shift fork.
does it grind on high RPM shifts??? if your hydraulics are getting gummed up the problem will be much more evident at high RPM's than at idle.
one more thing you may need to do is align your shifter. after the miles your shifter base may have gotten "tweaked" its kind of a bugger to get to as you have to remove your center console /radio bezel, then remove the 4 nuts holding the rubber shifter silencer. there should be an alignment "angle pin" that should drop down into the hole without any problems. if it doesnt go in then you will have to put the shifter in neutral, undo the 4 mounting bolts, and align the shift so that the pin will drop in. then tighten the 4 bolts again.
Also... another quick tip for you... put the car in reverse BEFORE you start the car.. when my car was cold it would do the same thing on the fresh rebuild when the fluid was really cold. Putting it in reverse before you start it will be easy and create much less wear on the transmissions internal parts.
